Please study the levels of 11 to 14 childcare provision described below. We would be grateful if you could indicate your likely usage of the facility by placing ticks in the appropriate boxes.

BREAKFAST SESSIONS 8am – 8.30am

To be run by two supervisory staff
Light breakfast provided
Non active activities available
Cost including breakfast - £1.50 per session. (See point 4 in letter regarding claiming back the cost.)

To be run and supervised by two qualified Youth Workers

Chill out facility

Cost including snack - £9.00 per ½ day session. (See point 4 in letter regarding claiming back the cost.)
